What’s the story with Fosamax?
Recent reports have women wondering if they should stop taking this widely prescribed osteoporosis drug.
In 1995, the FDA approved alendronate (Fosamax) for the treatment of postmenopausal osteoporosis, a bone-weakening condition that affects more than eight million women and causes 1.5 million fractures each year in the United States. Fosamax increases bone mineral density and significantly reduces the risk of spine, hip, and wrist fractures in women with osteoporosis and in those with low bone density that doesn’t meet the criteria for osteoporosis (a condition called osteopenia).
Fractures are an important cause of disability and death in postmenopausal women. Hip fractures lead to hospitalization and, usually, surgery — and they often result in nursing home care. Only 40% of hip fracture patients ever regain their independence, and nearly 25% die within a year. Vertebral fractures can cause debilitating back pain, and they, too, increase the risk of premature death.
Fosamax belongs to a class of drugs called bisphosphonates, which work by slowing resorption, the breakdown phase of normal bone remodeling. Fosamax is the oldest of these drugs and has been used the most and studied the longest. We know, for example, that it improves bone density for at least 10 years. Most patients tolerate Fosamax well; its most common side effects are irritation of the esophagus and stomach ulcer.
In the past few years, reports have emerged linking bisphosphonates with osteonecrosis (bone death) of the jaw and atrial fibrillation. With respect to Fosamax, those concerns have largely been allayed: users rarely develop osteonecrosis of the jaw, and the evidence for a relationship with heart rhythm problems is conflicting. But now, new concerns have been raised by reports of unusual fractures of the thighbone (femur) in long-term Fosamax users. There are no evidence-based guidelines on how long patients can or should take Fosamax, but some women and their physicians are considering a drug holiday. Many of our own readers have written, asking if they should stop taking the drug.
What’s the evidence?
In 2008, researchers in Singapore published a report on 17 postmenopausal women, average age 66, who experienced fractures across the thighbone unprovoked by major trauma. All of the women had been taking Fosamax for an average of five years, and 13 of them had leg pain before the fracture developed. The researchers reported similar findings in a smaller study in 2007; they theorized that prolonged suppression of bone remodeling by Fosamax may have encouraged fracture-inducing microdamage to the bone.
Prompted by the Singapore findings, clinicians at New York’s Hospital for Special Surgery identified 70 patients who had suffered low-energy fractures between 2002 and 2007. (Low-energy fractures occur from a fall from standing height or less.) Twenty-five (36%) had been taking Fosamax. Of these, 20 had suffered a fracture across the femur, and 19 of those fractures occurred in patients who had been taking Fosamax the longest — on average, seven years. The researchers concluded that long-term Fosamax use is a significant risk factor for low-energy fractures of the femur.
The initial reports also drew letters to the editor of The New England Journal of Medicine, including evidence both supporting and refuting a link between bisphosphonates and nontraumatic fractures. The studies thus far have been small, retrospective analyses, and they haven’t taken into account other factors that could contribute to such fractures, including general ill health. But given what we know about the effects of bisphosphonates on bone remodeling, the findings seem plausible.
In the short term, slowing bone resorption increases bone density. But in the long run, it may impair new bone formation and reduce the bone’s ability to repair microscopic cracks from normal wear and tear. (There’s some evidence in animal studies that Fosamax can inhibit microdamage repair.) Over time, such microdamage might accumulate and cause a fracture. Also, while bone breakdown is suppressed, the mineralization process continues, potentially resulting in “hypermineralized” bone, which may be more brittle and less resilient to wear and tear. This is all largely speculative, as no studies have produced empirical evidence that such mechanisms actually lead to fractures.
Now what?
Merck, the manufacturer of Fosamax, says it will undertake further study of the drug’s effects on bone. And the FDA and Merck will continue monitoring for adverse events. In the meantime, it’s important not to overreact. Fosamax has a proven capacity to prevent fractures — and the disability and death that can accompany them. By 2010, according to the National Osteoporosis Foundation (NOF), as many as 12 million Americans will have osteoporosis, and over 40 million will have osteopenia (low bone mass). Most experts believe that when Fosamax is used appropriately, its benefits greatly outweigh the risks.
Clinicians diagnose osteoporosis by measuring bone mineral density with a technique known as dual-energy x-ray absorptiometry (DXA). DXA results are used to calculate a statistical measure called a T-score. Experts recommend that a woman with osteoporosis (defined by a T-score of –2.5 or below) should strongly consider drug treatment to reduce her fracture risk.
Women with osteopenia — a T-score between –1.0 and –2.5 — should consider other risk factors before deciding about drug therapy. The World Health Organization has developed the Fracture Risk Assessment Tool (FRAX), an online calculator that estimates an individual’s 10-year risk of having a hip fracture or other major fracture (available at www.nof.org andwww.shef.ac.uk/FRAX). The NOF suggests that a woman with osteopenia should consider drug treatment if the FRAX calculator says that her 10-year risk for a hip fracture is at least 3% — or her 10-year risk for any major fracture is at least 20%.
Currently, there’s no consensus on how long you should take a bisphosphonate medication. Until we know more, women taking Fosamax who have severe osteoporosis or a prior fracture should probably continue doing so indefinitely. But results from the Fracture Intervention Trial Long-term Extension (FLEX) study published in 2006 suggest that some women can eventually stop or take a break. In that study, women who had taken Fosamax for at least five years were randomly assigned to continue the drug or switch to a placebo for five more years. Those who discontinued Fosamax (the placebo takers) showed a gradual decline in bone density and a slight increase in the risk for spine fractures, but the rate of hip fracture, a far more serious injury, was the same in the two groups. (Women with severe osteoporosis — a T-score below –3.5 — were excluded from the study.)
What can you do?
As with any drug, don’t take Fosamax unless you’re sure you need to. If you’ve been taking it and are concerned about long-term effects, talk to your clinician about taking a break. Unfortunately, we have little solid evidence to guide us in this area. We know that bisphosphonates stay in bone for years, so it’s not clear that a “drug holiday” will lower your risk for possible long-term effects. If you decide to take a break, be sure to have your bone density tested after a year or two. If it has declined significantly, you can always resume bisphosphonate therapy.
Meanwhile, continue all the other measures that help protect and maintain bone density: take 1,200 to 1,500 milligrams of calcium and 800 IU of vitamin D every day; get 30 minutes of weight-bearing exercise at least three times a week; and if you smoke, do your best to stop.

Readers: Here is another instance of a drug company using the unknown public to experiment on. This drug was issued supposedly to prevent women from having brittle bones, but instead it makes the woman’s situation worse by making certain bones in her body so brittle they break without experiencing any trauma. If she had done nothing, her only concern would be falling down and breaking a bone.
Now that doesn’t even need to happen. Long term use of this drug causes a woman’s bones to break without her having to fall or experience any kind of trauma to her bones. The killer is, for those who have been taking it for a few years, stopping may not help because the drug remains in their systems for years, weakening their bones year after year.

Can some types of osteoporosis drugs actually hurt my bones?

Certain types of osteoporosis drugs have been associated with an increased risk of two rare but serious problems — osteonecrosis of the jaw and an unusual type of fracture in the upper thigh bone (femur).
Osteonecrosis of the jaw develops when the jawbone fails to heal after a minor injury, such as getting a tooth pulled. An impending atypical femoral fracture can cause pain in the thigh or groin that begins subtly and gradually worsens. It sometimes develops in both legs at once. If untreated, a complete fracture of the femur requiring surgery can occur even with normal weight-bearing.
Bisphosphonates — such as alendronate (Fosamax, Binosto), risedronate (Actonel, Atelvia), ibandronate (Boniva) and zoledronic acid (Reclast, Zometa) — and denosumab (Prolia, Xgeva) have been linked to osteonecrosis of the jaw and atypical femoral fractures.
The risk appears to increase with the length of time the drugs are taken.
In addition to treating osteoporosis, bisphosphonates and denosumab are also used to treat cancer that has spread to the bone. The risk of osteonecrosis of the jaw is much greater for people taking higher doses of these drugs to treat cancer than it is for people who are simply treating osteoporosis.

Trump is selling alpha-male strength and power. It’s a hoax, of course. Trump is just a garden-variety bully. But the media aren’t trained to expose this kind of hoax. In fact, the more Trump can bulldoze and belittle his interviewers, as well as Hillary Clinton, the more he appears to show strength and power.
Because I’m very short, I was always bullied as a kid. I discovered that the best defense against bullies was to taunt them into revealing the weaknesses and insecurities that had made them into bullies.
After watching him for the past year, I’d guess Trump’s weakness and insecurities have to do with his not feeling very intelligent, not feeling respected in the circles in which he craves respect, and not feeling he’s the man his father wanted him to be. So questions from the media (and comments from Hillary) that provoke him in these areas will, I believe, cause him to expose the sham of his alpha-male strength and power.

This is another drug in which a drug company invented a need to sell the drug.
When Fosamax was released in 1995, it was heavily marketed to women to cure conditions called osteoporosis and osteopenia. While osteoporosis is a serious bone disease that leads to increased bone fractures, osteopenia is a much less serious thinning of the bones.
The drug maker, Merck, was instrumental in millions of men and women starting treatment for osteopenia. Before Fosamax, the disease was not considered serious enough for treatment. Shortly after the drug became popular, more and more complaints about the drug’s side effects surfaced. One of them was random bone fractures.

As a white woman who has been bullied by white men who dislike my dating asian men, I can attest to their inferiority complex when it comes to sexual prowess.
I have had relations with more white men than asian. In my 23 years of dating I have had serious relationships with 6 white males and 2 asian men.
None of the white men had anything to offer me but their money. In bed they were as useless as drill without a bit. I discovered this when I discovered the sensuality my body was designed to receive.
White males play mind games with white women to keep them from seeking out other sexual partners of a different race.

Acting macho is just one of them. Denying OTWs equality in the work place sends a message to white women that if they want to be taken care of they had better pick a white man.
The same is true about the legal system. Allowing cops to disrespect black men is another subtle message to white women that if they want to feel safe they need to be with a white man.

Good news (well, fingers crossed)! The United States Department of Justice announced it will be temporarily halting construction of the Dakota Access Pipeline effective immediately.
Michelle, please cover this story if you are able to. Sending in guard dogs to attack peaceful protesting is outrageous. The media is more than happy to cover Kapernick but where is the media now, when they should be protesting this outrage.

#5 Robert, Trump suffers from the dark triad which presents to many including you as being or pretending to be alpha. The dark triad is the mix of narcissism, Machiavellianism, and psychopathy. It can look like alpha or as he’s a decisive powerful man who speaks his mind’ in this case. Their mind does not process thoughts and information the same as others, the 2 top revealing symptoms in his case is overblown sense of self/grandiosity and lack of empathy (and he qualifies for many other top traits of the triad).
Straight up alpha males often do have some narcissistic traits (most people have a few traits bit to way lesser degrees than full blown narcs) and alphas usually exude confident and excel at business (and therefore attract the ladies, often they create triads in relationships if they take their ‘alphaness’ to the dark triad place) cross over to dark triad for even a nice alpha can occur … Just because … They can.
It doesn’t change the fact that ‘alpha’ is overall quite appealing, (even with women if they know how to wear it well : ) … Sadly, if women do not wear it well they get called every name in the book
Trump has been dark triad and a covert and often overt bully for a long time. That won’t change if he get this presidency and that’s why I said several times already, he’s DANGEROUS.

One correction…usually alphas don’t actually care what others think of them and capable of being good leaders and partners, narcs on the other hand usually not think of others at all (Trump and focus on money, go fact check what he actually said about the Iraq war) and in relationships they’re known for ‘love bombing’ i.e. Loving every hair on your head blah blah blah until they don’t often with no notice, don’t get me going, I’m thinking of a app/book something so women can handle it when it occurs, and not attract it (or fall for it again) narcs have uncanny abilities to tap into what you need to hear and Trump is a perfect example, look how many people are hearing what he’s saying and falling under the spell he creates, he is a crafty one who plays word salad very effectively (look at some of yesterdays comments) ~ ZL

Thanks for the drug info I’ll pass it along, I personally do not go to western docs (exception: that broken collarbone in march) If you were here 5-6 years ago and follow the eating and workouts early and consustently you can almost always avoid doctors advising you to take drugs you can live without. *barring some inherited disease, even then you can often control some triggers but it’s work instead of docs and pills, so it’s not for everyone.

If you believe that this pipeline can stop a leak in three mintues after it starts you are foolish. Unless they have someone living on site observing 24/7, it will first have ot be noted by someone then they have to get to the locations, then maybe it wll take 3 minutes to turn off the valve.
There is a glut in oil production many of the wells in Dakota are shut down. There is already at least one pipeline that crosses the Dakotas. Any more are not needed. they just want to connect up to the Canandian line.
We already have a huge pipeline leank in Yellowstone as well as on in the Dakotas already. This company needs to invest its money in alternative energy. But i suppose that means they would have to be a real company with more than a few token employee. God forbid they should use any of their proftis to employ people in hte area they are using or pay taxes in that region.
That might get in the way of someones at hire levels having a bonus, a new car , a vacation or a jet. Stop buying these corportists lies and half truths. They areonly in this for a quick buck at your expense.

DC District Court Judge James E. Boasberg, 53, is set to rule on the fate of the Dakota Access Pipeline on Friday. The clean drinking water of 20,000,000 of his countrymen hangs in the balance–an historic responsibility. But if history tells us anything, it’s that class allegiance is rarely, if ever, bucked.
As reported by Democracy Now “Bank of America, HSBC, UBS, Goldman Sachs, Wells Fargo, JPMorgan Chase and other financial institutions have, combined, extended a $3.75 billion credit line to Energy Transfer Partners, the parent company of Dakota Access.”
With such a significant investment from banks already designated “too big to fail” would a member of the monied class ever rule against his own?
Boasberg attended St. Albans, a private boy’s school in the nation’s capitol located in the shadow of the National Cathedral. The prep school’s motto is Pro Ecclesia et Pro Patria, for Church and Country. Its tuition is currently: Grades 4–12: $42,484*, Boarding School for grades 9–12: $59,892*
*New students also pay a one-time, non-refundable registration fee of $1,850 in addition to stated tuition.
Student Ethnicities (per Zillow) reveal a negligible student participation by young First Nations Peoples.

He crossed the Atlantic for Oxford where he earned an M.St. in Modern European History. The current fee for that program for overseas students is 17,555 British Pounds, or $23,548 per annum.
James then went to Yale Law School (his father had attended Harvard Law) where yearly tuition alone is currently $57,615. With the other ancillaries that figure rises to over $80,000 a year.
After receiving his J.D. he clerked for a year in the U.S. Court of Appeals for the Ninth Circuit, where the 2016 typical Law Clerk salary is $68,173. (Per Glass Door)
He went back to San Francisco, where from 1991 to 1994 he was employed as a litigation associate at Keker and Van Nest; salaries currently range from $211k – $251k. (Per Glass Door)
Then he left San Francisco to return to Washington, DC, and from 1995 to 1996 was an associate at Kellogg, Huber, Hansen, Todd & Evans, which according to “Above the Law” pays base salaries that are well above market: $225K for the first two years at the firm, followed by $275K after that. All associates who join the firm get a starting bonus of $175K.
He joined the Public Sector as an Assistant US Attorney in 1994. The current salary for that position is $137,086. (Per Glass Door)
He became a Superior Court Associate Judge in the District of Columbia where, according to a report in Legal Times , “judges have earned $174,000 annually since 2008, making them consistently among the highest paid general trial court judges in the country.”
Now he’s a District Judge, appointed by President Obama in 2011; his 2106 salary is $203,100.
Contrast Judge Boasberg’s lifelong financial privilege with the reality of the members of the Standing Rock Sioux Reservation. From Page 9 of the tribe’s Strategic Plan:
POVERTY: Widespread poverty is a major issue at Standing Rock, and is related to every obstacle addressed thus far. Both Corson and Sioux counties are “persistent poverty” counties, meaning that 20% or more of their population has lived in poverty over the last thirty years. The average 30-year poverty rate for the two counties is 42%. This kind of continuous cycle of poverty can be difficult to address.
OPPORTUNITY: Following a carefully considered Comprehensive Economic Development Strategy such as this one, which addresses many factors of the poverty cycle including employment, health, education, housing, infrastructure, and environmental concerns, will help begin to combat the cycle of poverty at Standing Rock.
The numbers presented by the American Indian Relief Council offer even more detail:
Many residents live in remote areas, far away from medical care and healthy food. Housing, both in remote areas and in towns, is in short supply, forcing many families to live in overcrowded conditions. Two out of three tribal members are jobless and residents’ annual income averages only $4,421.
Dissonances which beg the question: In the case of Persistent Privilege v. Persistent Poverty, who will prevail?

Donald Trump has finally accepted that Barack Obama was born in the United States, according to adviser Rudy Giuliani and Trump campaign manager Kellyanne Conway.
The former New York mayor made the remarks during a heated exchange with Chris Matthews on MSNBC’s “Hardball” on Thursday night.
“Donald Trump believes now that [Obama] was born in the United States,” Giuliani said. “I believe it, he believes it, we all believe it. But it took a long time to get it out.”
Trump had been one of the most prominent voices in the so-called “birtherism” conspiracy — the consistently debunked rumor that Obama was born in another country.

I really hope that a debate moderator asks Donald this question because he will never admit Obama’s US citizenship otherwise–for two reasons
1. Very rarely does Donald actually admit that he was wrong, let alone apologize for it. This situation is especially problematic because Donald state he had hired investigators to look into the birth certificate, and thus he would also have to admit that he never hired them, or that they never found anything after all and that he was lying about one or the other.
2. If he admits at this late stage, he’d loos some support among the racist right who have been supporting his dog whistles about Obama’s birth since 2011.
